Nurse Practitioner - Auburn Crest Primary & Palliative Care

About the role

Auburn Crest Primary & Palliative Care is hiring a Nurse Practitioner to manage a panel of primarily geriatric patients in their homes across a designated region. You will practice at the top of your license under Idaho's full practice authority, with an interdisciplinary team behind you and roughly an hour with each patient.

What your days look like

You will see patients in their homes, care facilities, and assisted living communities, with real autonomy and a team behind you.

  • Perform comprehensive in-home assessments: history, symptom review, physical exam, medication reconciliation, and depression screening
  • Complete vitals and focused exams including monofilament testing, urine dipstick, and foot exams as appropriate
  • Diagnose, treat, and document, using your clinical judgment to build an accurate problem list
  • Identify gaps in care and communicate findings clearly back to the patient's PCP and the interdisciplinary team
  • Educate patients and families on disease process, medications, and what to expect next
  • Practice at the top of your license under Idaho's full practice authority
  • Protect patient privacy and maintain HIPAA compliance in every setting


What you bring
  • Active, unencumbered Idaho nursing license
  • NP certification through ANCC, AANP, or AACN as Adult, Adult-Gerontology, or Family
  • One or more years of clinical experience at your highest level of education, or three or more years as an RN
  • Ability to obtain DEA licensure and prescriptive authority post-hire
  • Reliable transportation and a valid driver's license, with comfort covering a designated territory
  • Genuine ease with seriously ill patients, difficult conversations, and families in hard moments


A plus, not a requirement: internal medicine background, home health or hospice experience, and strong organizational habits.
