Responsibilities

  • Provide medical care for terminal illnesses in accordance with the state Nurse Practice Act under physician supervision
  • Conduct history and physical examinations to evaluate patient health
  • Complete hospice face-to-face visit requirements for eligibility assessment
  • Recommend certification and recertification for hospice care to the medical director
  • Maintain accurate clinical documentation
  • Engage in interdisciplinary meetings to address patient and caregiver needs
  • Ensure compliance with all relevant legal and regulatory requirements

Responsibilities
  • Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
  • Completing history and physical examinations by evaluating the patient's medical condition and health history
  • Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
  • Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
  • Completing and maintaining accurate clinical notes
  • Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
  • Ensuring compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned
Qualifications
  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
  • Current DEA license, if applicable to state regulations
  • Current nursing license in state of practice
  • Current CPR certification
  • Master’s degree with minimum of one year NP experience
  • One year hospice or palliative care experience preferred
  • Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
  • Commitment to clinical and documentation excellence
