Position SummaryThe
Nurse Navigator coordinates diagnostic and treatment processes and follows up at assigned Asante facility(s). Guides patients and their families through the continuum of care and collaborative process with medical care providers and ancillary support programs. They act as a central point of contact for all medical care providers working with the patient, and facilitate evidence-based nursing care. They assess educational needs and design, teach, and/or coordinate educational programs for patients, families, and the community.
QualificationsEducation- Bachelor of Science in Nursing, or equivalent combination of education and experience, is required
- Master's Degree in Nursing, preferred
Experience- Minimum 3 years of clinical nursing experience, including bedside patient care, is required
Licensure/Certifications- RN: Registered Nurse licensed by the Oregon State Board of Nursing is required upon start
- BLS: Basic Life Support for the Healthcare Provider certified by the American Heart Association (AHA) is required upon start
- OCN: Oncology Certified Nurse by Oncology Nursing Certification Corporation (ONCC) is preferred
- ONN-CG: Oncology Nurse Navigator Certified Generalist by the Academy of Oncology Nurse Navigators (AONN) is preferred
- ONS/ONCC Chemotherapy Immunotherapy Certification is preferred
