JOB DUTIES- Provide clinical leadership for Care at Home Services nursing teams and oversee day-to-day operations, staffing, and patient assignments.
- Collaborate with physicians, pharmacists, advanced practice providers, case management, supply chain, and other interdisciplinary team members to coordinate patient care and operational services.
- Collaborate with clinical and operational leadership to support patient care, quality, safety, and organizational objectives.
- Serve as a clinical resource to staff, providers, patients, and families regarding Care at Home Services.
- Support operational workflows related to patient scheduling, clinical coordination, supply readiness, emergency response planning, and care delivery.
- Provide leadership support for Care at Home Services operations outside of standard business hours, including participation in on-call responsibilities and response to urgent clinical, staffing, operational, or patient care needs as required.
- Ensure compliance with nursing standards, plans of care, professional nursing practice requirements, and organizational policies and procedures.
- Ensure timely, accurate, and complete clinical documentation and compliance with applicable CMS, Joint Commission, state, and organizational requirements.
- Ensure Care at Home Services operations comply with CMS Conditions of Participation and other regulatory requirements and serve as the Primary Clinical Coordinator/Nurse Supervisor as assigned.
- Promote adherence to the ANA Code of Ethics, Joint Commission standards, and nationally recognized standards of nursing practice.
- Maintain continuous survey readiness and participate in regulatory surveys, audits, accreditation activities, and corrective action planning.
- Monitor quality indicators, patient outcomes, and operational metrics and lead performance improvement initiatives in collaboration with quality teams.
- Lead patient safety, risk reduction, occurrence reporting, and incident review activities and ensure implementation of corrective and preventive actions.
- Participate in routine and special patient care conferences and interdisciplinary reviews.
- Participate in recruitment, hiring, onboarding, orientation, training, and performance evaluation of clinical staff.
- Maintain oversight of staff competency, licensure, certifications, orientation requirements, annual competencies, and continuing education to ensure safe and compliant patient care.
- Promote teamwork, engagement, accountability, and professional development within an interdisciplinary care environment.
- Provide coaching, counseling, performance management, and disciplinary actions, up to and including termination, in accordance with organizational policy.
- Meet or exceed strategic financial goals through effective management of departmental budgets, staffing plans, and operational resources.
- Develop and manage annual capital and operational budgets in collaboration with nursing and administrative leadership.
- Identify, implement, and evaluate efficiency and cost-saving initiatives and communicate recommendations and outcomes to leadership.
- Manage payroll review, attendance records, specialty equipment utilization, and other departmental resources to optimize operational efficiency and patient outcomes
- Manage daily staffing operations, including staffing assignments, staffing huddles, workforce planning, productivity monitoring, and reporting of staffing effectiveness measures, vacancy rates, turnover rates, and workforce trends.
- Collaborate with Human Resources and recruitment teams to support workforce planning, recruitment, retention, and staffing effectiveness initiatives.
- Collaborate with nursing and operational leadership to support program growth, service expansion, patient access, strategic initiatives, and new service development.
- Travel throughout the service area and to patient homes, clinics, and other care locations as necessary to provide staff support, clinical oversight, competency validation, operational coordination, patient care observation, and clinical practice review.
