Responsibilities

  • Supervise, mentor, and support nursing staff, providing guidance and immediate feedback.
  • Assess resident needs, promote independence, and facilitate person-centered care.
  • Lead and promote the Immerse philosophy of human-centered care within Memory Support.
  • Monitor staff performance, ensure timely completion of duties, and respond to emergencies.
  • Administer medications, treatments, and direct nursing procedures accurately.
  • Collaborate with the interdisciplinary team, physicians, and families to coordinate care plans.
  • Maintain accurate documentation, ensure regulatory compliance, and uphold resident rights.

Memory Support Program Manager/Unit Manager - Rydal Park, Jenkintown

Rydal Park, a premier Life Plan Community, is seeking a compassionate and highly motivated Memory Support Supervisor to oversee the clinical and non-clinical operations of our Memory Support neighborhood. This role is responsible for ensuring high-quality resident care, team member performance, resident engagement, and family satisfaction. The ideal candidate thrives in a collaborative environment, builds strong relationships with staff, provides real-time coaching and feedback, and leads a multidisciplinary team committed to person-centered dementia care, regulatory compliance, and exceptional service.

Schedule: Full-time, Monday - Friday, 8:00 am - 4:30 pm, typically on call every 5th weekend

Pay: starting at $97,000.00 based on experience.

Key Responsibilities:
  • Supervise and support Memory Support team members, providing coaching and guidance.
  • Lead the Immerse philosophy of person-centered dementia care.
  • Assess resident needs and ensure quality, individualized care.
  • Administer medications, treatments, and nursing procedures.
  • Collaborate with residents, families, physicians, and interdisciplinary team members.

  • Respond to resident care concerns, emergencies, and changes in condition.
  • Promote resident engagement through partnership with Life Enrichment.

  • Maintain accurate documentation and ensure regulatory compliance.

Qualifications & Skills:
  • Current Pennsylvania LPN license in good standing required.
  • Experience in memory care, assisted living, long-term care, or a related healthcare setting preferred.
  • Prior leadership or supervisory experience preferred.
  • Strong communication, organization, and problem-solving skills.
  • Experience with EHR systems; PCC preferred.
  • Ability to manage multiple priorities while maintaining a resident-centered approach.
