Job Summary:We are seeking a compassionate, dynamic
Medical Director to provide overall clinical leadership and executive oversight for an established Program of All-Inclusive Care for the Elderly (PACE). In this dual administrative and clinical role, you will direct medical care policies, lead the Quality Assurance Performance Improvement (QAPI) program, and directly supervise Primary Care Physicians (PCPs) and Nurse Case Managers.
This position is ideal for a board-certified physician with a dedicated background in geriatrics or complex chronic disease management who thrives in a participant-centered, collaborative interdisciplinary environment.
Key Responsibilities:Executive Clinical Leadership & QAPI:- Clinical Supervision: Manage, mentor, and supervise Primary Care Providers (PCPs) and Nurse Case Managers across clinical sites and partner nursing facilities.
- QAPI Oversight: Direct the Quality Assurance Performance Improvement (QAPI) program, leading clinical audits, policy updates, medical protocol standardizations, and medical grievance reviews.
- Interdisciplinary Collaboration: Serve as the senior medical authority on interdisciplinary care teams, driving comprehensive care planning, preventive health strategies, and resource allocation.
Direct Participant Care & Coordination:- Clinical Assessments: Conduct participant intake evaluations, manage complex chronic conditions, and address acute or emergency medical needs as they arise.
- Advance Care Planning: Engage in proactive advance care planning discussions with participants and families, managing care goals and end-of-life decisions with empathy and cultural sensitivity.
- Continuum Coordination: Oversee medical care transitions across center day rooms, acute hospitals, short-term rehab, and nursing facilities.
- Call Coverage: Participate in an executive physician on-call rotation (typically 2 one-week assignments every 3 to 4 months).
Candidate Requirements & Qualifications:Education & Specialty Board Certification:- Degree: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution [Required].
- Board Certification: Board Certified in Internal Medicine or Family Practice[Required].
- Subspecialty: Board Certification or added qualifications in Geriatrics[Highly Preferred].
Licensure & Credentials:- State Licensure: Active, unrestricted State Medical License [Required].
- Federal/State Registrations: Active Controlled Substance License (CDS) and DEA Registration [Required].
- Life Support: Active Basic Life Support (BLS) certification [Required].
Experience & Attributes:- Senior Care Experience: Minimum of 1+ year of documented clinical experience working directly with a frail or elderly population [Required].
- Supervisory Track Record: Proven leadership or administrative supervisory experience directing physicians or multi-specialty clinical staff [Required].
- Core Competencies: Expertise in chronic disease management, EMR optimization, and multidisciplinary care alignment.
