RN MDS Coordinator to oversee resident assessment and reimbursement processes while partnering with the interdisciplinary team to support quality care. This position offers a consistent weekday schedule with
no weekends and no on-call responsibilities.
Position OverviewAs the RN MDS Coordinator, you will manage the resident assessment process from start to finish, ensuring documentation accuracy, regulatory compliance, and timely completion of all required assessments. Your work will help ensure residents receive appropriate care while supporting accurate reimbursement for services provided.
Key Responsibilities- Lead and manage the MDS/RAI assessment process for assigned residents
- Review clinical documentation to ensure assessments accurately represent resident conditions and care needs
- Monitor assessment schedules and deadlines to maintain compliance with federal and state regulations
- Collaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plans
- Support Medicare, Medicaid, OBRA, and payer-specific assessment requirements
- Conduct chart reviews and documentation audits to promote reimbursement accuracy
- Assist with quality improvement initiatives and survey preparation activities
- Serve as a resource for clinical staff regarding MDS and assessment-related processes
QualificationsRequired- Active Registered Nurse (RN) license in the State of Illinois
- Graduate of an accredited nursing program
- Minimum of two years of MDS 3.0 coordination and case management experience in a skilled nursing, rehabilitation, or post-acute care environment
- Strong organizational and communication skills
- Proficiency with Microsoft Office applications
Preferred- Experience working in long-term care or skilled nursing settings
- RAC-CT certification, or ability to obtain certification within six months of hire
Compensation & Benefits- Annual salary: $87,800 - $99,700, depending on experience
