Responsibilities

  • Direct and manage completion of MDS assessments and care plans

Full Job Description
This position is eligible for a $2,000 Sign-on Bonus working 40 hours a week, Monday-Friday (payout starts after 6 months of employment).

Position Summary:

Directs and manages the completion of the Resident Assessment (MDS), Triggers and RAPs in accordance with current rules, regulations and guidelines that govern the resident assessment. Directs and supervises the development of full, quarterly and comprehensive assessment and care plans of each resident within required time frames. Development of monthly MDS calendar and communication of scheduled assessments with the Interdisciplinary Team members. Participate in weekly Standards of Care meeting. Coordinates and conducts facility daily PPS meeting as well as weekly/monthly/ Triple Check processes to ensure timely communication of clinical/RUG data to the Business Office for reimbursement processes. Manages and coordinates physician certification and recertification files, and ensure that certs/recerts are done timely in conjunction with Medical Records. Assists and coordinate coding of ICD-9 codes in accordance with resident disease process/skill need. Participates in the Facility Quality Improvement (CQI) process. Serves on the committees and develops/implements improvement. Adheres to established HIPPA confidentiality standards of patient/resident and client information. Assist with monitoring ADLs coding conducted by the nursing assistants. Performs all other tasks or duties as assigned.

Professional Experience/Educational Requirements: Registered Nurse preferred with demonstrated experience in Long-Term Care and RAI/PPS processes.

Hours: Monday-Friday- 8:00-4:30 (some week-ends required to open assessments as needed)
