Full Job Description

Join a growing cardiovascular program and help shape the future of heart failure care in a dynamic, patient-centered practice. This position offers the unique opportunity to help the growth of a young Heart Failure (HF) program with strong institutional support and a well-established referral network for advanced heart failure therapies, including LVAD and heart transplantation.

The practice is equipped with comprehensive cardiovascular imaging capabilities, including advanced echocardiography, cardiac CT, cardiac MRI, nuclear cardiology, and PET, providing the tools needed for comprehensive heart failure evaluation and management.

Key Responsibilities:

  • Help the growth of the existing regional Heart Failure program.

  • Provide comprehensive care for patients with acute and chronic heart failure across the continuum of disease.

  • Focus on early identification, intervention, and disease prevention strategies to reduce heart failure progression and hospitalizations.

  • Collaborate closely with general cardiologists, interventional cardiologists, electrophysiologists, primary care providers, and multidisciplinary teams.

  • Utilize advanced multimodality cardiovascular imaging to support diagnosis, risk stratification, and management decisions.

  • Partner with established advanced heart failure centers to facilitate seamless referral for patients requiring LVAD, transplant evaluation, or other advanced therapies.

  • Expand community outreach, education, and referral relationships to increase access to high-quality heart failure care throughout the region.

Must have completed an accredited graduate medical training program and be board certified or eligible within the area of medicine stated. In addition, must have or be able to obtain a license to practice in our multi-state region with no restrictions or limitations. Medical staff in our system will supervise advanced practice clinicians. Medical staff may need to provide outreach service as part of their role in the system. Teaching students, residents, fellows or clinical staff is cornerstone to our Medical Education philosophy and an expectation of all medical staff.
