Position Summary

University Health Network (UHN), West Park Healthcare Centre Site is looking for an experienced professional to fill the key role of Manager of Professional Practice (MPP).

UHN is a leader in adult rehabilitation and complex continuing care. It brings together innovative patient care, groundbreaking research and diverse education to help people overcome or recover from disabling injury, illness or age-related conditions. A Reactivation Centre/Transitional Care Program provides short-term, goal-oriented interdisciplinary care for patients who no longer require acute hospital services but are not yet ready to return home. Care focuses on rehabilitation, reconditioning, functional improvement, medical management, and discharge planning to maximize independence and support a safe transition to the community or another care setting.

Reporting to the Senior Director of Professional Practice (SDPP) and working closely with the Lead Manager of Professional Practice, this Manager of Professional Practice role will provide professional practice leadership to UHN's Transitional/ Reactivation Programs. The individual will be a regulated health care professional, a member of the UHN Collaborative Academic Practice (CAP) portfolio as well as a member of the Transitional Care Program Leadership Team. The successful applicant will support the development, implementation and maintenance of profession-specific and interprofessional practice standards at the program-level and across UHN, as required.

Duties
  • Collaborate with Directors of Professional Practice, Managers of Professional Practice (MPP), Clinical Directors, Program Managers, Patient Care Coordinators (PCCs), and other nursing and health professions practice leaders to ensure that practice standards are aligned and integrated, where possible, across programs, and organization wide
  • Directly supervise Clinical Educators/APNEs across the portfolio, and maintain strong linkages with UHN's Practice-Based Education Office
  • Establish, maintain and lead a number of quality improvement initiatives and work closely with other practice and operational colleagues
  • Promote a vision of practice excellence, and integrate practice-based education and point-of-care research capacity within and across UHN programs, hospitals and in the external community
  • Mentor of diverse staff and learners is also an important aspect of the role
  • Support and partner with the SDPP and Lead MPP to develop and implement care process changes, develop standards of work; inform organization-wide care delivery models; and deliver on practice-based education priorities-all which are in service of creating a safe, exceptional patient experience and a quality, growth-producing environment for our staff
  • Actively engage in organization-wide projects, most notably the optimization of UHN's new health information system


Qualifications
  • Current registration with a Regulated Nursing or Health Professions College and able to practice without restrictions required
  • Completion of Master's Degree in Nursing/Health Professions, Education or Health Administration required
  • Qualifies for cross-appointment with the University of Toronto or other UHN affiliated College or University
  • A minimum of 5 to 7 years related clinical experience
  • A minimum of 3 years in a clinical leadership role
  • Experience in developing, implementing and maintaining professional practice standards and quality control measures in a health care environment required
  • Project management experience considered an asset
  • Demonstrated skills in managing financial, reporting and budgeting an asset
  • Consulting, influencing and negotiating skills
  • Ability to think strategically and at an organizational and systems level, as well as at the program, site, unit and individual levels
  • Broad knowledge of overall hospital structure, operating policies and decision-making processes
  • Knowledge of the Regulated Health Professions Act, standards of professional practice and codes of ethics, health care consent and ethical implications of care delivery
  • Comfortable with use of technology; knowledge of computer systems and software applications
  • Broad understanding of the health care system, related issues and trends, policies and legislation provincially, nationally and internationally
  • Excellent knowledge of quality improvement principles and practices
  • Sound analytical, critical-thinking, and problem-solving skills
  • Excellent judgment and decision-making skills
  • Strong verbal and written communication and presentation skills
  • Ability to collaborate effectively with diverse management and employee groups and individuals
  • Ability to adapt, implement and manage changes and facilitate its acceptance by others
