Job Title: Kaiser Case Manager - Licensed Vocational Nurse (LVN)

Position Summary

  • Job Type: Full-Time
  • Schedule: Day Shift (typically Monday - Friday)
  • Compensation: $45.00 - $47.00 per hour (Depending on experience)
  • Location: On-site at 16412 Los Gatos Blvd, Los Gatos, CA 95032

Key Responsibilities

  • Care Coordination: Manage patient care plans for assigned Kaiser Permanente residents from admission through discharge.
  • Utilization Review: Complete regular utilization management reviews using standardized criteria (e.g., InterQual) to optimize appropriate lengths of stay and resource allocation.
  • Interdisciplinary Collaboration: Act as the central point of contact between Kaiser health plans, facility physicians, therapy departments, nursing staff, and families.
  • Discharge Planning: Coordinate safe transition plans, arrange home healthcare/durable medical equipment (DME), and prevent unnecessary hospital readmissions.
  • Documentation & Compliance: Maintain precise, timely case management documentation in compliance with federal, state, and managed care contract guidelines.


Qualifications & Requirements
  • Licensure: Active, unrestricted California Licensed Vocational Nurse (LVN) license.
  • Experience: Minimum of 1-2 years of nursing experience; prior case management, utilization review, or managed care experience (specifically working with Kaiser systems) is highly preferred.
  • Skills: Strong organizational capacity, exceptional communication skills, computer literacy, and an ability to navigate complex medical files.
  • Knowledge: Familiarity with skilled nursing facility (SNF) regulations, Medicare guidelines, and insurance authorization processes.
