Full Job Description
Objective/Overview
The Integrated Care & Community Health Program Manager provides dual clinical and programmatic leadership for CUHCC's Integrated Behavioral Health-Care Coordination (IBH-CC) team and the Community Health Team, composed of community health workers (CHWs). This role combines direct patient care with operational oversight and clinical supervision. The Program Manager ensures high-quality, trauma-informed, culturally grounded behavioral health and community health services within a highly diverse primary care clinic.

Reporting Relationships
This position reports to the Mental Health Director and directly supervises the following staff: 1.0 FTE IBH licensed clinician, 2.0 FTE care coordinators, 2.5 FTE Community Health Workers and clinical trainees in practicum.

Work Environment
  • This is a hybrid position. Primary work environment is on site at the CUHCC clinic during clinic hours (generally 8am - 5pm, Monday - Friday). This role may come into contact with patients who may have contagious illnesses.
  • Telemedicine remote work will be available 1 day a week based on clinical and programmatic needs.
  • This position requires a negative TB test, and a Hepatitis B vaccination.

Job Responsibilities
Clinical Responsibilities (60%)
  • Provide traditional outpatient therapy services under the Outpatient Therapy program; may provide integrated behavioral health clinical services in primary care and dental settings.
  • Deliver trauma-informed, culturally grounded care addressing mental health, behavioral health, and overall well-being.
  • Serve as a primary clinical responder for crisis situations in the clinic, conducting risk assessments for patients who are suicidal, homicidal, actively psychotic, or emotionally dysregulated and may pose a risk to themselves, staff, or other patients; collaborate closely with the IBH clinician, mental health supervisors, and other clinical staff to ensure timely and coordinated interventions.
  • Collaborate with primary care providers, dentists, dental hygienists, clinical pharmacists, nutritionists, CHWs, and care coordinators to provide comprehensive, patient-centered care.
  • Meet individual clinical productivity and billable targets while maintaining high-quality care.


Supervision & Care Coordination Management (15%)
  • Manage the IBH-CC team and internship/practicum program, including hiring, onboarding, training, professional development, and performance management.
  • Provide clinical supervision to the IBH clinician, who delivers evidence-based, short-term interventions in primary care and dental settings, and conducts risk assessments; offer consultation and oversight on high-risk cases.
  • Oversee the care coordination team (bachelor's-level social workers or related qualifications), ensuring effective support for patients' health-related social needs (housing, food, transportation, access to care) as well as medical, behavioral health, and dental needs; care coordinators also facilitate referrals to higher levels of care (e.g., Targeted Case Management, IRTS, Help me grow) as needed.
  • Monitor team performance and productivity, generate reports, and lead quality improvement initiatives to strengthen program outcomes.

Community Health Team Oversight (15%)
  • Manage the CHW team including hiring, onboarding, training, professional development, and performance management.
  • Oversee the CHW team providing community outreach, one-on-one patient support, health education, skill building, home visits, and assistance with social needs.
  • Serve as primary liaison between CHWs, Quality Director, Pharmacy Director and CUHCC service lines to integrate CHWs into quality and care initiatives.
  • Develop and maintain CHW workflows, monitor productivity, and generate reports to support strategic priorities and grant requirements.
  • Collaborate with the Director of Advancement to coordinate CUHCC's participation in community outreach, place-partnerships, and events with external organizations.

Program Leadership & Administration (10%)
  • Oversee program operations for IBH-CC, CHW, and care coordination teams, ensuring alignment with organizational priorities, quality standards, and performance metrics.
  • Facilitate collaboration across primary care, behavioral health, and dental services to promote whole-person, team-based care.
  • Lead program evaluation, quality improvement initiatives, and reporting for internal and external stakeholders.
  • Demonstrate advanced communication, interdisciplinary collaboration, and systems-based thinking to support team performance and community engagement.

Qualifications

Required Qualifications
  • Masters degree plus at least four years of experience.
  • Independently licensed mental health provider (per state statute).
  • Board-certified clinical supervisor.
  • Masters-level clinical training with experience delivering trauma-informed, culturally grounded outpatient therapy and integrated behavioral health services.
  • Experience managing Community Health Worker (CHW) programs, care coordination services, or multidisciplinary teams.
  • Demonstrated ability to meet clinical productivity and program performance targets.
  • Strong leadership skills with experience in program development, staff supervision, and data-driven decision-making.
  • Demonstrated knowledge and application of trauma-informed approaches.
  • Experience working with diverse communities and individuals with significant health-related social needs.

Preferred Qualifications
  • Experience in a Federally Qualified Health Center (FQHC) or similar primary care-embedded setting.
  • Background in community-based programming and outreach.
  • Experience collaborating with integrated care teams, including CHWs.
  • Knowledge of and experience with CHW principles and best practices.
  • Knowledge of integrated behavioral health and care coordination models.
  • Familiarity with Health Care Home or other integrated, team-based care models.
  • Experience with health promotion programming, community outreach, and grant reporting.
  • Demonstrated ability to incorporate cultural understanding into interventions tailored for diverse populations.

Pay and Benefits

Pay Range: Between $86,000 - $93,000/annually for 1.0 FTE, depending on qualifications. Salary range is based on experience and overall mastery/job proficiency level of position.
