Facility Name: St. Elizabeth Hospital

Schedule: Full-Time
Start Bonus: $5,000

CME Allowance: $2,000

What You Will Do

This is an exceptional opportunity for a motivated Nurse Practitioner or Physician Assistant to join an established, clinician-led team at St. Elizabeth Hospital.
  • Collaborative Care Model: Join a tight-knit, established team of four daytime physicians and two daytime APPs, offering a supportive environment for professional growth.
  • Lead Patient-Centered Care: Diagnose and treat acute and chronic conditions, ensuring every individual receives a clear path toward recovery and long-term wellness.
  • Manage the Clinical Journey: Drive the full continuum of care-from initial assessment and diagnostic interpretation to preventative screenings-to identify health risks early.
  • Create Collaborative Treatment Plans: Coordinate seamlessly with physician partners and a robust network of specialists, ensuring patients never navigate their health journey alone.
  • Empower Patients & Families: Use proactive education and counseling to give patients the tools and confidence to manage their health effectively outside the hospital.
  • Document Excellence: Capture the patient story with precision and care, maintaining high-quality records that serve as a reliable roadmap for the entire care team.

What You Will Need

Licensure / Certification / Registration:
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
  • Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
  • Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:
  • Master's degree of Nursing required.
