Qualifications

• Active Washington State Registered Nurse (RN) license
• Prior Home Health and/or Hospice experience strongly preferred
• Current BLS certification from the American Heart Association
• Valid driver's license and insured, reliable vehicle

Responsibilities

• Provide skilled nursing care and symptom management to terminally ill patients
• Develop and manage individualized care plans in coordination with the hospice team
• Educate and support patients and families through end-of-life care
• Administer medications and treatments for comfort and quality of life
• Coordinate care with physicians, social workers, aides, and other team members
• Respond to urgent needs and provide crisis support when necessary
• Maintain accurate and timely documentation in the EMR
• Manage a caseload, scheduling and prioritizing visits based on patient needs
• Participate in interdisciplinary team meetings and plan-of-care updates
• Ensure compliance with hospice regulations and standards
