Responsibilities

  • Conduct face-to-face encounters for patients in their third or subsequent benefit period and document findings.

Join Our Team: Nurse Practitioner

At CVHHH, you won't be rushed through back-to-back appointments or constrained by the clock. Instead, you'll have the opportunity to slow down, connect deeply, and truly influence patient outcomes and quality of life.

Responsibilities:
  • Conducts face-to-face encounters for patients in their third or subsequent benefit period, documenting and sharing findings with certifying physician promptly.
  • Conducts well-patient checks.
  • Conducts symptom management visits.
  • Provides physician support.
  • Coordinates and collaborates with IDT members, community physicians, facility staff, and others as needed.
  • Participates in developing Plan of Care for assigned patients.
  • Assists in enhancing care strategies with Hospice Clinical Manager and Medical Director.
  • Acts as a resource for Hospice staff to ensure continuity and improve patient care quality.
  • Attends IDT meetings.
  • Provides backup support to Hospice Medical Director, covering medical needs of hospice patients as required
