SUMMARY
The Director, Nursing Hospice (DON) assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care. They may perform Clinical Manager duties and back up to the Executive Director (ED), as needed. The DON provides oversight for case management, clinical care and is responsible for coordination all clinical disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. The DON provides direct supervision to field staff and internal clinical support functions to achieve clinical excellence outcomes, regulatory compliance, patient and employee satisfaction, in a fiscally responsible manner. The DON understands and practices all legal and company rules and regulations.
KEY RESPONSIBILITIES
Oversees the overall clinical process for quality patient care from admission to discharge
Directly supervises the clinical staff, all disciplines who provide direct patient care, and the internal clinical support teams regarding day-to-day patient activities, clinical issues and concerns
Interaction with individuals (internal clinical teams, patient family member, physician, etc.) as applicable via phone, email and/or face to face concerning patient care
Organize and allocate clinical staff to carry out the activities necessary to accomplish program objectives
Assumes accountability for documentation and clinical charting practices in accordance with agency protocol and Medicare/Federal /State guidelines
Assists with the intake, referral and admission process as needed including but not limited to record review
Implement policy, procedures and quality improvement programs in the field
Monitors patient trends and outcomes through leadership of QAPI program
Monitor data submitted to Medicare for accuracy and implements corrections as warranted
Maintain professional growth and development through seminars, workshops and professional affiliations to keep abreast of the latest trends in field of expertise
Supports the selection of field staff
Assists with the on-boarding of new hires as well as on-going mentoring of employees to facilitate staff retention
Supervises field staff and clinical support and training staff with consistency and clarity of expectations
Performs audits, reports and analysis of reports related to the hospice organization
Oversees QAPI program
Ensures resolution of pertinent staff issues
Works closely with Care Integration partners to engage community through clinical leadership
Completes yearly evaluations in an accurate and timely manner
Performs coaching, counseling and discipline as needed
Maintains timely and accurate documentation of all aspects of quality tracking and employee coaching, discipline and education
Maintains a professional appearance and Cultural Fit as a representative of the company
Understands and practices agency policies and procedures
Other duties as assigned

MANAGEMENT RESPONSIBILITIES
Interviewing, hiring, training and termination of department personnel.
Provides consistent coaching and provides challenge and opportunity to direct reports.
Supervises direct reports with consistency and clarity of expectations.
Completes yearly evaluations in an accurate and timely manner.
Performs coaching, counseling and discipline as needed.
Works with direct reports to ensure employee satisfaction and resolution of issues and concerns.
Conducts employee performance reviews.
QUALIFICATIONS
This position requires a Registered Nurse in the state of employment
A minimum of one year of supervisory or administrative experience in hospice, or related health programs
Expertise and Hospice background for multiple sites preferred
Knowledge of guidelines governing hospice agencies
Able to handle financial reports and understand direct cost controls
Ability to create positive impressions and maintain effective communication with a variety of people
Knowledgeable on nursing best practices
Ability to identify a situation and handle it with the best possible solution
Detail-oriented and observant
Strong ability to multi-task and be adaptable as priorities change throughout the day
Operational knowledge of system software including but not limited to: EMR, PDA9s, payroll, Microsoft Office & Outlook preferred
