What You Will DoAs an RN Case Manager, you will provide skilled nursing care in the home while guiding each patient's plan of care from start to finish.
- Assess, plan, implement, and evaluate individualized patient care
- Develop care plans in collaboration with physicians and the interdisciplinary team
- Complete admissions and ongoing nursing assessments
- Educate and support patients and families through changing needs
- Document care accurately and in a timely manner

Salary Range: $72,000 to $90,000 annually, based on experience
What You Bring
- Active Missouri RN license in good standing
- Home health RN experience preferred
- Strong clinical judgment and documentation skills
- Ability to manage a flexible schedule including admissions
- Comfort with technology and electronic medical records
