Licensed Practical/Vocational Nurse (LPN/LVN) - Home HealthLocation: North and South Orange CountyJob SummaryWe're looking for a skilled and compassionate LPN/LVN to provide in-home nursing care under the supervision of a Registered Nurse (RN). You'll support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services.
Responsibilities- Provide nursing care according to the RN-developed Plan of Care.
- Monitor and document changes in patient condition; report to RN/physician as needed.
- Assist with activities of daily living to promote patient independence.
- Educate patients and caregivers within LPN/LVN scope of practice.
- Maintain accurate EMR documentation.
- Participate in case conferences and coordinate care with the home health team.
- Follow infection-control, HIPAA, and company policies.
Required Qualifications- Current LPN/LVN license in the state of practice.
- Graduate of an accredited practical/vocational nursing program.
- Current CPR/BLS certification.
- Valid driver's license, reliable insured vehicle.
- Strong communication, clinical judgment, and ability to work independently.
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
Physical Requirements- Ability to lift/transfer patients and perform physical caregiving tasks.
- Ability to pass required physical exams and annual PPD testing.

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $50 to $55 visit commensurate with experience and depending on type of visit.
