In this role, you will provide Assess and Evaluate patients in their homes for admissions to home health care services. You will create and develop individualized care plans, coordinate with physicians and interdisciplinary team members, educate patients and families, and help individuals achieve greater independence and improved health outcomes. Full-Time, Part-Time and Per diem Admission Nurse positions available.

Does This Sound Like You?You may be a great fit if you have:
- A current Connecticut RN license
- At least one year of nursing experience
- Strong assessment, clinical, organizational, and communication skills

- An interest in community-based healthcare
Home health experience is preferred, and experience with MatrixCare is a plus.
