Clinical Nurse Liaison with
Dartmouth Health Home Care, you'll continue working in the hospital collaborating directly with providers, discharge planners, and care teams-but with a new focus: ensuring a smooth, well-coordinated transition from hospital to home.
In this role, you'll draw on your acute care expertise to assess needs, build trust with patients and families, and coordinate with multiple partners to streamline the referral and discharge process. You'll still be part of the fast-paced hospital environment, but with a better schedule and less physically demanding responsibilities-allowing you to use your skills in a sustainable, impactful way.

The Clinical Nurse Liaison:
- Reviews provider orders, medical records, and related materials to determine patient clinical needs; uses clinical judgment to determine patient eligibility for services.
- Educates referring providers, care managers, discharge planners, patients, and families on the scope of services offered by VNH programs.
- Performs informational visits to prospective patients and families.
- Continues professional development by participating in continuing education programs, conferences, and workshops.
Qualifications - Demonstrated knowledge of current home health and hospice clinical standards of practice required
- Home health or ambulatory experience preferred.
- Knowledge of Vermont & New Hampshire State regulations, accreditation-based standards as applicable, and Medicare Conditions of Participation preferred.
Required Licensure/Certifications- Current or eligible to apply for RN license in both VT and NH or a compact license required. - Valid driver's license and reliable transportation is required.
