In this role, you will provide home safety evaluations, assess the patient's current and prior level of functioning, develop an individualized treatment plan, and guide the patient towards greater independence and improved health outcomes. Treat a diverse patient population including post-operative joint replacements, neurological, cardiac, respiratory, renal, and oncology patients. Coordinate with physicians and interdisciplinary team members, educate patients and families, and help individuals achieve their goals. Full-Time, Part-Time and Per Diem physical therapy positions available.

You may be a great fit if you have:
- A current Connecticut PT license
- At least one year of physical therapy experience
- Strong assessment, clinical, organizational, and communication skills
- A passion for patient education and empowering individuals to manage their health
- The ability to work independently while collaborating effectively with physicians and interdisciplinary team members
- An interest in community-based healthcare
Home health experience is preferred, and experience with MatrixCare is a plus.
