Health and Wellness Director

Agemark Senior Living | Sacramento, CA | Full-Time

This is a hands-on leadership role for someone who loves seniors, knows how to build and support a great team, and takes pride in creating an environment where residents receive exceptional, personalized care.

What You'll Do
  • Lead from the Front: Be present, approachable, and involved. You'll stay connected to residents and your team while setting the standard for excellent care.
  • Know Your Residents: Complete assessments, understand each resident's unique needs, and create personalized service plans that support safety, dignity, and independence.
  • Build a Great Team: Coach, mentor, and develop wellness staff while creating a culture of accountability, teamwork, and genuine care.
  • Connect the Dots: Keep residents, families, team members, physicians, and healthcare partners informed and working together.
  • Own Clinical Excellence: Oversee nursing services, documentation, service plans, and care practices to ensure quality and consistency.
  • Keep Medications on Track: Provide oversight of medication management and ensure medications are handled safely and accurately.
  • Stay Ahead of Risk: Identify changes in condition and residents who may need additional support, then put proactive plans in place.
  • Be There When It Matters: Respond to resident needs and emergencies calmly and confidently, ensuring proper follow-up and communication.
  • Keep Us Survey-Ready: Maintain accurate documentation and ensure wellness practices meet California requirements and community standards.
  • Make Wellness Meaningful: Champion education, prevention, and wellness initiatives that help residents live healthier, more fulfilling lives.

What You Bring
  • Licensed & Ready: An active LVNlicense in California.
  • Senior Care Experience: At least 2 years of experience working with seniors and a strong understanding of their health and wellness needs - REQUIRED
  • Leadership Experience: At least 2 years of experience leading, coaching, or managing a team - REQUIRED.
  • People Skills: You communicate clearly, build trust easily, and know how to navigate conversations with residents, families, employees, and providers.
  • Strong Judgment: You can assess a situation, solve problems, and make thoughtful decisions when it matters most.
  • Organized & Tech-Savvy: You're comfortable managing documentation, details, deadlines, and day-to-day technology.
  • Flexible & Dependable: You understand that senior living is a 24/7 environment and can support evenings, weekends, and Manager on Duty responsibilities as needed.
