Health and Wellness Director Astoria Senior Living | Oakdale, CA | Full-Time
What You'll Do
Lead & inspire your team: Be a visible, hands-on leader who mentors, trains, and supports wellness team members while building a culture of Excellence and Team Spirit.
Oversee resident care: Develop and maintain individualized service plans based on resident needs, assessments, and physician orders.
Build strong relationships: Serve as a trusted partner for residents, families, team members, physicians, and healthcare providers.
Maintain quality & compliance: Ensure resident records, service plans, documentation, and nursing services meet state regulations and Agemark standards.
Assess resident needs: Evaluate prospective and current residents to ensure we can safely and successfully meet their care needs.
Oversee medications: Promote safe and accurate medication management while monitoring for potential concerns.
Respond when it matters: Provide hands-on support when needed, respond to resident emergencies, and ensure appropriate follow-up and documentation.
Promote resident wellness: Identify potential risks and lead proactive programs that support residents' health, independence, safety, and quality of life.
What You'll Bring
Active RN or LPN license valid in California.
At least 1 year of experience working with seniors and a strong understanding of their health and wellness needs.
At least 2 years of leadership or management experience.
Strong knowledge of medications, dosages, potential side effects, and safe medication practices.
Excellent communication, organization, problem-solving, and leadership skills.

Flexibility to work evenings or weekends and serve as Manager on Duty as needed.
