COMPENSATION Annual Pay Range: $79,424.01 - $100,603.28
Hourly Pay Range: $38.184620 - $48.366961

JOB DETAILS AND QUALIFICATIONS Clackamas County's Housing and Community Development Department is seeking a mission-driven professional to help advance the integration of housing and healthcare systems across our community. As the
Health and Housing Systems Integration Program Planner, you will play a key role in designing, implementing, and improving programs that connect individuals experiencing housing instability with critical healthcare and supportive services.
Reporting to the Health and Housing Systems Integration Program Supervisor, you will lead the planning, coordination, and evaluation of programs and partnerships that bridge homeless services, healthcare, and other systems of care. This includes strengthening connections between Permanent Supportive Housing (PSH) programs and physical and behavioral healthcare, supporting the implementation of a new Medical Respite program, and coordinating Health-Related Social Needs (HRSN) services across internal departments and external partners.
In this role, you will evaluate programs, identify opportunities to improve system coordination, and develop strategies that enhance access to integrated, person-centered services. You'll collaborate with Health, Housing and Human Services (H3S) staff, healthcare providers, community-based organizations, Metro counties, Coordinated Care Organizations (CCOs), the Oregon Department of Housing and Community Services (OHCS), the Oregon Health Authority (OHA), and other regional and statewide partners to strengthen housing and healthcare integration throughout Oregon.

In addition, the ideal candidate will demonstrate:
- Experience working within housing, healthcare, Medicaid-funded programs, or other human services systems.
- Knowledge of the Homeless Management Information System (HMIS) and experience using data to evaluate programs, measure outcomes, and support reporting.
- An understanding of Oregon Medicaid, Coordinated Care Organizations (CCOs), Health-Related Social Needs (HRSN), and Long-Term Services and Supports (LTSS).
- Experience researching, evaluating, and improving programs through policy analysis, program evaluation, and performance measurement.
- Strong contract management and program oversight skills, including monitoring performance, ensuring contract compliance, and partnering with providers to improve service delivery.
- Excellent communication, collaboration, and relationship-building skills, with the ability to work effectively across government agencies, healthcare organizations, community-based organizations, and regional partners.
Required Minimum Qualifications/ Transferrable Skills:* - A minimum of four (4) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
- Demonstrated experience advancing racial equity in a workplace and/or community setting
- Experience with the development and implementation of strategic plans and/or programs related to housing or healthcare
- Knowledge of principles, methods, and procedures of local, regional and statewide planning
- Ability to facilitate meetings and provide trainings with county and community partners
- Experience analyzing and interpreting data using Microsoft Excel to evaluate program performance, identify trends, and support data-informed decision-making.
Preferred Special Qualifications/ Transferrable Skills:* - Experience with Medicaid or related healthcare programs, including Medicaid waivers and billing Medicaid for services.
- Experience with homeless services or related programs, including using the Homeless Management Information System (HMIS) for data collection, reporting, and program evaluation.
- Experience in program evaluation and analysis of program design and administration
- Experience with systems evaluation

TYPICAL TASKS Duties may include but are not limited:
- Coordinate regional and statewide partnerships to develop and implement innovative programs that integrate healthcare and housing systems, including initiatives supporting the Medicaid 1115 Housing Benefit.
- Develop and recommend program policies and strategies that strengthen housing and healthcare integration while collaborating across divisions to advance system-wide implementation.
- Evaluate program performance and outcomes by planning and conducting system monitoring, measuring effectiveness, and identifying opportunities for continuous improvement.
- Research system barriers and recommend solutions by engaging with internal staff, healthcare providers, community partners, and contracted organizations to improve coordination and service delivery.
- Represent the department on regional workgroups and strategic planning efforts by collaborating with Metro counties, Coordinated Care Organizations, the Oregon Health Authority, and other cross-jurisdictional partners to advance shared housing and health initiatives.

WORK SCHEDULE This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off).
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
This position is eligible for
hybrid on-site/telework and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
