Description
We are currently searching for a
Family Nurse Practitioner or Physician Assistant to help build and grow our brand-new
Urgent Care in Middletown, NY.
ANNUAL SALARY: $130,000 annually
WORK LOCATION(S): Middletown, NY
STATUS: Full Time
The Position:- The position offers an exciting opportunity to help establish a new urgent care while working in a supportive, mission-driven environment. The provider will work as part of a two-person clinical team, delivering efficient, patient-centered care for acute illnesses and minor injuries across the lifespan.
- While Cornerstone providers collaborate closely, this role is ideal for an experienced FNP or PA who is confident practicing independently, making sound clinical decisions in a same-day care environment.
- Schedule:
- 4 - 10-hour workdays. Site will be open Monday through Saturday. Schedule rotates each week but requires working every other Saturday.

Requirements
- NYS licensed Family Nurse Practitioner or Physician Assistant
- Active DEA
- Board Certified/Eligible
- Current AHA BLS/CPR
- Minimum 3 years of clinical experience (urgent care, emergency medicine, or primary care)
- Ability to manage patients across the lifespan
- Strong clinical decision making and procedural skills
- Commitment to community health and underserved population
