Full Job Description
This position requires traveling to the following clinics:
  • Angleton
  • Alvin
  • Lake Jackson
  • Clute
  • Freeport
Your Responsibilities
  • Assess, diagnose, and manage the healthcare needs of patients across the lifespan.
  • Obtain comprehensive health histories through patient interviews, family discussions, and physical examinations.
  • Identify health risks and develop appropriate treatment plans based on clinical findings, diagnostic testing, and patient assessments.
  • Provide wellness assessments, preventive care, and chronic disease management.
  • Counsel and educate patients on health conditions, treatment plans, and health-promoting activities.
  • Collaborate with physicians and other healthcare professionals to deliver coordinated, whole-person care.
  • Refer patients with unstable or complex conditions to specialty care when appropriate.
  • Maintain complete, timely, and accurate patient documentation.
  • Ensure prompt follow-up regarding laboratory results, referrals, medication needs, and patient inquiries.
  • Support quality improvement initiatives and contribute to positive patient health outcomes.
  • Maintain professional licensure and continuing education requirements.
  • Perform other duties as assigned.
What You'll Need
  • Licensed Family Nurse Practitioner with recognition as an Advanced Practice Registered Nurse (APRN) or licensed Physician Assistant recognized by the Texas Medical Board
  • Master of Science in Nursing
  • Certification in area of practice or eligibility for certification
