POSITION SUMMARYWorks in collaboration with the Chief Medical Director and Hospice Team to provide Hospice care services to geriatric and pediatric patients and families within the scope of practice of a nurse practitioner. Perform procedures for which he/she is privileged to do and document assessments, evaluations, consultation reports, and progress notes.
ESSENTIAL FUNCTIONS- Conduct Face-to-Face visits to assess patients' need for ongoing hospice eligibility.
- Complete required documentation to support eligibility and to meet regulatory requirements for communication of finding to Community Hospice of Texas / Providence Hospice Medical Director.
- Communicate to RN Case Manager any patient needs requiring medical intervention.
- Coordinate visits and scheduling with the Patient Care Manager.
- Maintain compliance with all regulatory and accrediting agencies.
- Responsible for demonstrating an appreciation and understanding of the philosophy, mission, and service standards of Community Hospice of Texas and for providing excellent customer service.
POSITION QUALIFICATIONS- Licensed as an Advanced Practice Nurse in the State of Texas
- At least 2 years of experience in geriatrics or hospice
- Excellent organizational skills
- Proficient skill using a computer is required; Microsoft Office Suite experience is required.
