Full Job Description
Position Title: ECM Site Manager

Salary: $76,000-$90,000 / annually, DOE + Monthly Performance Bonus

Lead a team that makes a meaningful impact on the lives of high-risk, high-need Medi-Cal members. As the ECM Site Manager, you will oversee the daily operations of an Enhanced Care Management (ECM) program, managing both onsite and remote team members while driving operational excellence, accountability, and exceptional patient outcomes. This is a hands-on leadership role that blends people management, performance coaching, operational oversight, compliance, and collaboration with internal teams and community partners. The ideal candidate is an experienced healthcare leader with a strong background in Enhanced Care Management, proven supervisory experience, and the ability to motivate teams while maintaining high standards of accountability and compassionate care.

Responsibilities
  • Lead the day-to-day operations of an Enhanced Care Management (ECM) site
  • Manage, coach, and develop a team of Lead Care Coordinators and support staff
  • Monitor team productivity, call metrics, and operational performance using data-driven reporting
  • Ensure compliance with organizational policies, regulatory requirements, and program standards
  • Conduct interviews, participate in hiring decisions, onboard new employees, and support employee development
  • Complete performance evaluations, coaching conversations, and disciplinary documentation when necessary
  • Partner with internal departments, health plans, and community organizations to ensure exceptional service delivery
  • Travel periodically for training, oversight, and quarterly leadership meetings

Requirements
  • 5-7+ years of progressive healthcare leadership experience, including Enhanced Care Management (ECM)
  • Previous supervisory or management experience leading healthcare teams
  • Experience interviewing, hiring, coaching, conducting performance reviews, and managing employee relations
  • Bachelor's degree required (extensive relevant experience may be considered in lieu of a degree)
  • Strong proficiency with Microsoft Office Suite, including Excel formulas, Outlook, and Word
  • Comfortable using Microsoft Teams, Zoom, RingCentral, and applicant tracking systems
