JOB SUMMARY
The Director works under limited supervision within SWHR Information Resources to coordinate, support, and ensure population health applications effectively meet the needs of Southwestern Health Resources (SWHR).

EXPERIENCE AND EDUCATION

Required
  • Education
    Bachelor's Degree in related field
  • Experience
    10 years application implementation and support experience in an IT department of an acute care or ambulatory setting to include at least three (3) years in supervisory capacity.

Preferred
  • Education
    Master's Degree in health care administration, business administration, or information technology management
  • Experience
    Hospital or ambulatory services operational experience, or IT experience supporting these areas and products


JOB DUTIES
  • Responsible for the implementation, management and support of Population Health applications across the Health System.
  • Works in the following care settings: Ambulatory clinics, affiliated physician practices, long term care organizations, free-standing or affiliated surgical centers, cancer centers, etc.
  • Utilizes information technology to improve patient safety and improve performance improvement reporting.
  • Responsible for managing population health priorities with Health System and ACO Leadership.
  • Responsible for facilitating IT governance referencing population health across the Health System. This will require collaboration across existing IT Governance processes.
  • Responsible for review, development, recommendation, and implementation related to the Population Health program.
  • Recommends changes, reviews progress and approves proposals for the IT Population Health program.
  • Manages utilization of various resources to maintain maximum efficiency and meet performance objectives for organization.
  • Responsible for design, execution, and effectiveness of a system of internal controls, which provides reasonable assurance that operations are effective and efficient, assets are safeguarded, and financial information is reliable.
  • Ensures compliance with applicable laws, regulations, policies, and procedures. Maintains confidentiality as appropriate.
  • Oversees budget expenditures and scheduling.
  • Oversees supervisors and/or subordinate managers to include establishing priorities.
  • Performs other duties as assigned.
