Location: Remote. Travel to PACE programs as needed

Pay Range: $120,000 - $130,000 (Based on Experience)

Schedule: Monday - Friday 8am - 5pm

What You Bring (Requirements)

  • Bachelor's degree required, Master's degree in public health, Healthcare Administration, Nursing, or a related field preferred.
  • 3 to 5 years of proven experience in population health management, particularly in a managed care setting, with a demonstrated understanding of data analytics and disease management.
  • Certification in Population Health, Healthcare Quality (e.g., CPHQ, CPH), or a related credential is a plus.
  • Familiarity with electronic health records (EHR) systems and population health management tools.
  • Ability to work flexible hours. Ability to travel is required.


What You'll Be Responsible For

The Director, Population Health will lead and advance efforts to improve health outcomes for participants within VOANS PACE Organizations. The Director will be responsible for the development and execution of population health strategies that integrate data analytics, community partnerships, clinical operations and evidence-based care practices. This role will be pivotal in driving PACE quality improvements and managing costs while ensuring participant centered care plans for our participant population.

Strategic Leadership

  • Lead the development and execution of the VOANS Senior CommUnity Care population health strategy, with a specific focus on improving outcomes for participants.
  • Collaborate with Medical Directors, Clinical Leaders and Interdisciplinary Teams to define and prioritize health outcome improvements for PACE participants in each VOANS PACE Organization.
  • Align population health goals with organizational objectives to improve participant care, reduce utilization costs, and enhance participant satisfaction.


Data Analytics & Performance Management

  • Utilize data analytics to assess the health needs of PACE participants, identifying trends, risk factors, and opportunities for intervention, including addressing social determinants of health (SDOH).
  • Oversee the collection, analysis, and reporting of key population health metrics, ensuring data-driven decisions by clinicians and interdisciplinary teams to improve participant outcomes.
  • Identify and implement technology solutions that enhance population health performance and reporting capabilities, ensuring that the tools and platforms support clinical outcomes.


Clinical Leadership & Collaboration

  • Mentor and support Clinical Services Directors to foster high quality and regulatory compliant PACE Clinic, Home Care and Medication Administration.
  • Assist Clinical Services Directors to create seamless clinical systems with primary care providers.
  • Provide training, guidance, and support to clinical teams to ensure the successful implementation of clinical care strategies and initiatives.


Program Development & Implementation

  • Design, implement, and manage targeted population health programs, including chronic disease management, preventive care, and health education, ensuring these programs are tailored to the needs of PACE participants at each of the VOANS PACE Organizations.
  • Provide resources and tools to support VOANS PACE Organizational leadership with effective partnerships with community organizations, healthcare providers, and stakeholders to ensure comprehensive support for participants and their families and to generate VOANS programmatic outcomes.
  • Collaborate with VOANS Health Plan Operations Team to assist in maintaining effective reimbursement systems and outcomes.


Quality Improvement & Compliance

  • Collaborate with The Compliance and Performance Excellence Team to ensure that population health programs meet regulatory standards and contribute to continuous improvement initiatives.
  • Monitor and evaluate the impact of population health programs on clinical outcomes, patient satisfaction, and utilization.
