Responsibilities
- Develop and refine workflows to strengthen referral pipelines and stakeholder engagement, with the goal of maintaining and increasing referrals through efficient processes.
- Oversee referral assignment, outreach, enrollment, and case closure to ensure accurate weekly census reporting that supports data-driven operational planning.
- Lead staff training and compliance initiatives to align with DOH and organizational standards, ensuring all staff are knowledgeable and compliant with program requirements.
- Provide bi-monthly coaching to supervisors to support staff growth, case management quality, and leadership development through consistent feedback.
- Prepare the departments for audits and maintain documentation accuracy that ensures data integrity and regulatory compliance.
- Facilitate KPI review meetings to analyze performance data, identify trends, and implement action plans that enhance service delivery and outcomes.
- Partner with leadership in recruitment and onboarding to maintain adequate staffing levels that support program success and operational goals.
- Guide program billing reviews and revenue processes to ensure reimbursement accuracy and achieve financial performance targets.
Qualifications
- Master’s degree in Social Work, Psychology, Mental Health Counseling, (required).
- Strong knowledge of multi-systems in NYC, including healthcare, behavioral health, and social services.
- 5 plus years of experience working with children and families affected by chronic medical and/or mental health conditions.
- 3 plus years of supervisory and leadership experience, with the ability to mentor, motivate, and manage multidisciplinary teams.
- Experience with field work and home visits, demonstrating understanding of direct service delivery and family engagement.
- Strong time management and organizational skills, with the ability to manage multiple priorities in a fast-paced environment.
- Care coordination experience with a focus on integrated and person-centered approaches.
