Lead a High-Impact Wound & Ostomy Program in Home Health

We are seeking a Certified Wound, Ostomy, and Continence Nurse (CWOCN) to serve as a Program Manager in home health wound care and ostomy care leadership. This role provides clinical oversight, program development, and quality improvement for a growing wound care and ostomy care program within home health.

This position is ideal for a clinician with experience in wound care management, ostomy care, home health nursing, clinical education, and quality improvement who is ready to step into a nursing leadership and program development role.

Key Responsibilities (CWOCN Program Leadership | Wound & Ostomy Care Management)
  • Lead and manage a home health wound care and ostomy care program using evidence-based practice and WOCN guidelines
  • Implement and standardize wound care protocols, ostomy care protocols, and clinical practice guidelines
  • Provide wound assessment, ostomy assessment, and virtual/field clinical evaluations
  • Ensure adherence to wound care standards of practice and ostomy nursing best practices
  • Conduct clinical chart audits, documentation audits, and quality assurance reviews
  • Monitor clinical outcomes, wound healing rates, and patient quality metrics
  • Provide nursing education, staff development, and clinical remediation for wound and ostomy care
  • Collaborate with physicians, providers, and interdisciplinary teams on wound care orders and ostomy management plans
  • Oversee supply formulary management, wound care products, and ostomy supplies optimization
  • Evaluate and implement advanced wound care technologies, negative pressure wound therapy (NPWT), and innovative dressings
  • Participate in quality improvement (QI) initiatives, performance improvement projects (PIP), and clinical outcome improvement programs
  • Partner with leadership in home health operations, clinical compliance, and regulatory standards (CMS, Medicare home health)

Required Keywords / Qualifications

  • WOCN-accredited education program graduate
  • CPR certification
  • Minimum 2 years experience in home health nursing, wound care, or ostomy care
  • Experience with clinical supervision, nursing leadership, or case management preferred
  • Strong background in wound care management, ostomy care management, and patient assessment
  • Experience with chart audits, clinical documentation review, and quality improvement initiatives
  • Familiarity with Medicare home health regulations and CMS guidelines preferred
