Full Job Description
SUMMARY

Under general supervision, coordinates clinical pharmacy consulting services in support of long-term care patients and staff by recommending and monitoring patient drug therapy to enhance favorable patient outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITES:

  • Coordinates services related to clinical pharmacy practice and assists in long-term staff development related to these activities
  • Performs initial consultations; develops and coordinates therapeutic programs with physicians and nursing staff; monitors therapeutic programs and assesses drug therapy outcomes
  • Reviews patient diagnosis and drug profiles; consults with physicians regarding unclear orders
  • Provides drug information to nursing staff and conducts periodic in-service training on pharmaceutical products and their uses; responds to all related after hour calls in a timely and accurate manner
  • Periodically reviews, audits, and compiles pharmaceutical statistical reports and summary data for supported long-term care facilities
  • Assumes responsibility for own professional growth by participating in continuing educating activities and/or professional organizations
  • Complies with all HIPAA policies specifically relating to patient confidentiality and release of information; maintains strict confidentiality on all client information; attends staff in-services and training

SKILLS AND ABILITIES

  • Extensive knowledge of pharmaceutical principles and practices
  • Considerable knowledge of Geriatric pharmacy practice
  • Considerable knowledge of drugs, their uses and possible adverse drug reactions
  • Knowledge of Federal and State laws regulating pharmacy practice and the use of controlled substances
  • Must be able to demonstrate typing skills or computer terminal use proficiency
  • Ability to communicate to others the types, uses, and potential effects of drugs
  • Ability to effectively communicate and present information in a clear and concise manner, both orally and in writing
  • Ability to establish and maintain effective working relationships with fellow associates, administrators, patients and families, physicians, and the general public

MINIMUM QUALIFICATIONS :

  • Current licensure in good standing as a Registered Pharmacist in the State of Georgia
