Job Summary

The Complex Transitions Coordinator Lead provides lead work guidance and technical expertise to the Complex Transitions Coordinators. This position develops processes to support accurate and timely data collection and analysis, leads continuous improvement efforts, and supports cross-divisional and enterprise-wide collaboration to improve complex transition services and outcomes. The role also leads agency, interagency, and interdepartmental policy initiatives to strengthen coordination, continuity of care, and person-centered supports for people experiencing complex transitions. Responsibilities include:

• Provide lead work guidance to the Complex Transition Coordinators to prioritize, direct, and guide the work of the team, provide advanced technical expertise, and support collaborative solutions
• Develop methods and processes to assure the accurate and timely data collection by the coordinator positions. Produce and interpret data summaries and reports and bring that data to the cross division continuous improvement team, and to cross departmental and enterprise-wide meetings to identify sustainable ways to support complex transition processes and services
• Lead the development of agency, interagency and interdepartmental policy initiatives to ensure a continuum of services that support and strengthen services and programming for people with complex needs or transitions
• As assigned by management, manage projects, participate in collaborative efforts, and assist with other duties as assigned to support TT&T and the Aging & Disability Services Administration

Qualifications

Minimum Qualifications

3 years of professional experience in human services, public assistance, disability services, behavioral health or related policy/program administrations. Experience must demonstrate the following:

• Experience with federal and state laws, policies and funding for services supporting Minnesotans with disabilities and chronic conditions including Medical Assistance (MA), HCBS waivers, assessment tools and service agreements
• Extensive knowledge and understanding of issues that impact long-term care of older adults and people with physical developmental or intellectual disabilities, brain injury, complex health conditions, or mental illness; familiarity with related providers and advocacy organizations
• Experience facilitating, collaboration and building effective working relationships among agencies, providers, advocacy organizations, and other stakeholders
• Experience analyzing, developing, implementing, or evaluating policy, program, legislative, or system changes

Preferred Qualifications

• Strong knowledge of DHS reform initiatives, person-centered planning, and informed choice with experience developing, implementing, and evaluating policy, program and legislative changes; ability to advise others on policy alignment and systems change
• Familiarity with legislative processes, procedures, and protocols to inform policy implementation and program development
• Strong communication skills, verbal and written; experienced in drafting technical, policy, and narrative documents and presenting information through reports, charts tables and other formats
• Experience analyzing, developing, implementing, or evaluating policy, program, legislative, or systems changes
• Proficiency in Microsoft Office Suite, MMIS, SSIS and CRM; quick to learn and apply new technologies
• Variety of experiences working effectively with others from different backgrounds and/or cultures
