Primary Team Leader Responsibilities:- Create an environment that supports and advances professional practice, teamwork and communication through mentoring and role modeling application of mental health clinical knowledge
- In collaboration with the team and manager, organize and prioritize day-to-day operations of the team, staffing, scheduling and resource utilization
Round, monitor and address patient and family satisfaction with care - Facilitate staff orientation and identify staff learning needs and appropriate resources in collaboration with program Educators & Manager
- Support the accountability of team members to the Manager with regards to patient care, professional conduct and performance
- Participate in hiring, orientation and performance evaluation of staff
- Liaise with members of inter-professional team (inpatient and outpatient and Emergency Department) to ensure excellence in patient care
- Regularly completes chart reviews and audits and follow-ups to ensure adherence and compliance to standards of care
- Assists in the development of effective interpersonal relationships among interprofessional team through coaching, mentoring and facilitating difficult conversations
- Support standardized and timely patient assessments, transfers, and discharges to promote safe, transparent, equitable, and timely patient flow.
- Support hospital bed flow by collaborating with the Emergency Department, Bed Flow Coordinator, and bed flow management to develop patient flow strategies, facilitate complex discharges and transfers, and optimize access to inpatient mental health services.
- In addition to the above, this position will hold a reduced allocation of clinical time.
Primary Clinician Responsibilities:- Responsible and accountable for decisions and actions in the provision and management of patient care, providing crisis intervention services in the Emergency Department
- Coordinating follow up services with relevant community resources
- Practitioner, consultant and educator for appropriate patient groups and health care team members
- Provide written assessment and treatment recommendations
- Provide clinical teaching, mentoring and/or supervision for Social Work students; engaging with inter professional education experiences with students of all health disciplines
Qualifications: - Minimum five years current Mental Health experience, with children, youth and adults experiencing psychosocial issues, psychiatric conditions or mental health disorders, with 3 years of experience in a formal or informal leadership position within a healthcare environment.
- Masters of Social Work - in good standing with the Regulatory College
- Previous clinical mental health experience in an acute care hospital setting is required
- Sound knowledge of mental health disorders, DSM classifications and pertinent legislation
- Sound knowledge of mental status evaluations, psychosocial assessments, suicide and violence risk assessments, psychiatric history taking, and discharge planning
- Comfortable working within a multidisciplinary team, under pressure and within a changing environment
- Skilled working with a broad range of client groups and their significant supports
- Able to maintain timely and thorough documentation
- Working knowledge of community resources
- Excellent interpersonal, communication and organizational skills
