- Pay: $86,880 to $89,000 based on experience
- Schedule: M-F 8:30AM-5:00PM
- Visiting Client Territory: Buffalo area. Would need to be present in each specific area when the Manager of Clinical Services is unavailable for that area.

You will...- Performs on-site supervisory and teaching visits to evaluate caregivers
- Provides clinical orientation to caregivers on assigned home care cases.
- Participates in the training of new nurses and in-services for field staff.
- Performs initial nursing assessments
- Counsels patient/caregiver in meeting nursing and related needs.
- Assigns and coordinates services provided.
- Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs.
- Prepares clinical progress notes ensuring any changes in the patient's condition is reported
- Monitors clinical records for complete and accurate documentation.

- CPR Certified
- 1 year of Home Care experience required
- 1 year of RN experience in a skilled care setting
- Valid NYS Driver's License, insurance, registration, and reliable car
