Full Job Description
Scope of Work

Provides pharmaceutical care according to professional and departmental standards and statutory requirements. Provides safe, effective, rational, and cost-conscious pharmacotherapeutic recommendations through the department's medication management programs. The Clinical Pharmacist assists in supervising the functional activities of Pharmacy Technicians and Interns.  The clinical pharmacist has state of Michigan pharmacist in charge responsibilities for this pharmacy location including compliance to all regulatory requirements.

Maintains accountability for ensuring the safety and efficacy of medication use. Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations. Works with multiple medication automation systems.
  • Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions. Safely compounds and dispenses medications for administration to patients.
  • Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
  • Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
  • Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
  • Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
  • Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.

Qualifications

  • Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
  • 2 years of relevant experience hospital pharmacy Preferred
  • American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred
  • LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
  • LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required Or
  • CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required Or
  • CRT-Basic Life Support (BLS) - AHA American Heart Association 180 Days required
  • Ideal candidate will be a Pharmacist possessing PharmD Degree and/or PGY1 Residency and/or PGY2 in Retail / Community / Ambulatory Pharmacy
  • Ideal candidate will also possess strong understanding of retail or outpatient pharmacy operations, will proactively ensure compliance with requirements as well as critically evaluate internal processes for continuous improvement
  • As this role is expected to have significant growth in the next 6 months, ability to be flexible and to adapt to changing environment will be critical
  • Weekends would be as needed
  • This position would be On-Site for the most part but could have a very small component of Remote work once everything is in more of a sustainable phase
