Clinical Pharmacist - Med/Surg

St. Luke's Boise Medical Center is seeking a Clinical Pharmacist to join our Adult Medical/Surgical Pharmacy team. This position provides comprehensive pharmaceutical care for a diverse adult patient population across general medicine and surgical services.
Our Med/Surg pharmacists practice as integrated members of the patient care team, working closely with physicians, nurses, pharmacy technicians, dietitians, and other disciplines. The role offers a high degree of clinical independence and the opportunity to directly influence medication therapy, patient safety, clinical workflows, and pharmacy practice.
This is an excellent opportunity for a pharmacist who enjoys broad-based acute care practice, complex medication management, interdisciplinary collaboration, teaching, and continuous improvement.
What You'll Do• Provide comprehensive medication management for adult Med/Surg and Hospital at Home patients.
• Manage complex therapies including antimicrobials, anticoagulation, renal dosing, parenteral nutrition, pain management, and electrolyte replacement.
• Identify and resolve medication safety concerns, interactions, dosing issues, and opportunities to improve therapy.
• Support medication reconciliation, transitions of care, and discharge planning.
• Respond to medication-related questions from providers and nurses.
• Precept pharmacy residents and students.
• Contribute to guidelines, protocols, medication-use evaluations, Epic optimization, and other quality-improvement projects.
• Work primarily day shifts in Med/Surg, with occasional rotation into centralized pharmacy staffing, evening, and night coverage as needed based on departmental staffing needs, including every other weekend coverage.
- May assist in the training of technicians, externs, interns, residents and other department staff.
- Serve as a role model and mentor to technicians, students, residents and new practitioners.
- Perform other duties and responsibilities as assigned.
• Work primarily day shifts in Med/Surg, with occasional rotation into centralized pharmacy staffing, evening, and night coverage as needed based on departmental staffing needs, including every other weekend coverage.

Qualifications
- Education: Degree from an accredited pharmacy program
- Experience: 0 years' experience
- Licenses/Certifications: Valid Pharmacist License in state of practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
Preferred Qualifications
PGY1 residency, BCPS or other relevant board certification, or previous inpatient hospital pharmacy experience, preferably in a Med/Surg setting
