Position Description: Responsible for demonstrating an expertise in medication reconciliation and emergency medicine. Is able to serve as a resource in the provision of pharmaceutical care to patients in the emergency department.
Duties: - Medication Reconciliation is one role of the ED pharmacist. In collaboration with the nursing staff perform routine review and correction of home medication lists for patients admitted into the hospital via the ED.
- Interprets and verifies medication orders in a timely manner. Maintains accurate, complete pharmacy patient medication profiles.
- Maintains awareness of developments in pharmacy and health care that relate to job responsibilities and integrates them into their practice.
- Respond with the rest of the code blue team in whatever part of the hospital that a code blue is called. Manage the compounding and algorithmic monitoring of medications, manning the code cart allowing for nursing, respiratory therapy, and the physicians to focus on the patient specifically. Coordinate replacement of medications during code blue situations as needed.
- Respond to all code STROKE calls while on duty. Be on stand by for thrombolytic orders, to mix and ensure an accurate dose and administration within 60 mins.
- Respond to all code STEMI patients while on duty to provide any assistance and immediate verification of orders or putting in the orders at the bedside for the provider. Review for recommendation for aspirin on arrival
- Serve as a resource to assist nursing and the physicians in response to questions regarding:
Toxicology Respond to all overdoses to evaluate what was taken, the pharmacokinetics, the toxicology and antidotes if needed before poison control is contacted. Pediatric dosing and verification Pain medication administration for long bone fracture Behavioral health medication continuation upon admit or transfer Medication safety - Guardrail use - Performs other duties as assigned/required.

Qualifications Requirements: - Bachelors of Science in Pharmacy required. Pharm.D. preferred.
- Completion of a 1-year emergency pharmacy practice residency, specialty residency, emergency/trauma certification, or at least 3 years of pharmacy experience is strongly preferred.
- Current licensed pharmacist in South Carolina.
- ACLS, BLS, PALS certification required for ED/FED.
- Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics.
- Serve as resource to assist nursing and physicians in response to questions regarding toxicology; pediatric dosing, pain medication, behavioral health medication continuation and medication safety.
- Knowledge of computer applications. Pyxis experience with knowledge of Pyxis Reports.
