Excelat Woodbury for Rehabilitation & Nursing is seeking a dedicated Clinical Coordinator, RN to support high-qualityresident care through close monitoring of resident conditions, timely clinicalfollow-up, staff education, and collaboration with nursing leadership. Thisfull-time role is ideal for an experienced registered nurse with strong clinicaljudgment, excellent communication skills, and a passion for serving olderadults.
KEY RESPONSIBILITIES:
- Attend daily morning meetings to receive updates on residents experiencing changes in condition or requiring continued medical intervention.
- Closely monitor residents with changes in condition or clinical stability concerns raised by residents, families, or staff.
- Review labs, diagnostics, and other clinical information daily to help ensure appropriate intervention and follow-up.
- Support triage, assessment, and clinical interventions as needed or requested for residents.
- Communicate effectively with physicians, residents, families, and interdisciplinary team members regarding findings, interventions, and changes in condition.
- Collaborate with and educate nursing staff to promote strong clinical communication, continuity of care, and staff confidence.
- Round on residents admitted within the last 30 days and follow up on identified needs.
- Round on residents with communicable diseases or isolation needs that may increase risk of clinical decline, including influenza, RSV, COVID, early C. diff, norovirus, and other active infections.
- Ensure clinical documentation is completed when indicated to support the resident record.
- Provide clinical education when opportunities for staff development are identified.
- Attend STAT calls and assist with urgent clinical needs as appropriate.
- Identify clinical system concerns and communicate them to leadership.
- Collaborate with the Administrator and nursing leadership on key areas of focus
QUALIFICATIONS AND WORK ENVIRONMENT
• Valid New York State Registered Nurse license required
- Current CPR certification required.
• Strong desire to work with older adults and support resident-centered care.
• Strong clinical assessment, critical thinking, and follow-up skills.
• Excellent communication, interpersonal, stress management, and customer service skills.
• Ability to read, speak, and write English.
• Ability to multi-task, stay organized, and remain focused in an active clinical environment with frequent interruptions.
• Ability to maintain a high level of daily walking and activity, including resident rounding.
• Must demonstrate empathy, professionalism, and a calm approach when interacting with residents, families, visitors, and staff.
• Role may involve exposure to infectious disease, residents with behavioral needs, and family members or visitors experiencing frustration or concern.
