Job Description
Responsible for the planning and monitoring of patient care issues for all inpatients during their stay. Coordinates care and discharge planning with the physician and all other disciplines involved in the total care of the patient. Acts as a liaison for referrals / consults, patients and their insurance carrier, and assists in maintaining and improving quality of care. Is able to diagnose and treat human responses to actual or potential health problems through services such as case finding, health teaching, health counseling and provision of care supportive to or restorative of life and well-being and executing medical regimens prescribed by a licensed physician, or other licensed health care provider legally authorized to do so.
Education And Credentials
Graduate of an accredited school of Professional Nursing.
Experience
Two years of medical/surgical experience preferred. Charge nurse or other health related management experience preferred. Utilization Review and Discharge Planning experience preferred.
