Job Summary and Qualifications
Summary: The Case Management Manager shares responsibility for the planning, direction, coordination, and control of activities concerned with case management. Assists in the provision of resources and assists to remove barriers to successful operation of case management. .Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical, financial, and psychosocial approach through the continuum of care and balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent clinical review, patients will be assessed to determine appropriateness of admission, continued hospitalization, clinical resource consumption, proactive care management along the continuum of care, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues will also be monitored for and reported to the QRM department as a part of the CQI process.
Under the supervision of the Case Management Director, the Case Management Manager is responsible for the department's day to day operations, staff/scheduling/team building and performance improvement. The Manager will participate in the interviewing, monitoring, development, coaching, evaluation and disciplining process of staff. The Manager will demonstrate understanding of the budget process and cost-effective resource utilization. Reviews staff assignment, documentation and review of selected patients. When need arises, will perform other duties as assigned by the Director. Position Requirements:
Licensure/Certification/Registration: Current licensure in the state of Colorado as a Registered Nurse or Licensed Clinical Social Worker (LCSW). Current BLS certification through the American Heart Association or American Red Cross is also required.
Education: BSN required if Nurse, or Master's required of LCSW
Experience: At least three of case management experience required. At least two years previous experience in charge/supervisory or management position preferred. Able to demonstrate leadership and management skills.
SpecialQualifications: Must have the ability to assess information, apply critical thinking process to determine patient's progress. Requires excellent human relations and oral/written communication skills. Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations and team collaboration and patient interviews along with customer service skills. Must also have a strong analytical and organizational skills.
