Case Manager - Full Time - Thornton, Colorado

Denver Regional Rehabilitation Hospital, serving the greater Denver, Colorado area, provides specialized inpatient rehabilitative services in a warm and patient focused setting. We offer intensive therapy programs for patients recovering from strokes, neurological injuries, spinal cord and orthopedic trauma, and other debilitating conditions.   • Current clinical licensure strongly preferred.

  • Ability to maintain proper levels of confidentiality.
  • Ability to work closely and professionally with others.


Responsibilities

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital's mission and "Guiding Principles" into daily practice.
