Full Job Description
Job Summary and Responsibilities

As our RN Care Coordinator, you will help patients navigate the healthcare continuum so they can achieve optimal health outcomes and a smooth transition to post-acute care.

Every day you will assess, plan, facilitate, and advocate for options and services for assigned patients. You will be expected to effectively coordinate care, communicate with various stakeholders including physicians and insurers, and collaborate to ensure quality and cost-effective outcomes within the appropriate level of care.

To be successful in this role you must possess excellent communication and collaboration skills, a strong ability to advocate for patient and family needs, and a comprehensive understanding of utilization review and discharge planning processes.

Job Requirements

Required:
  • Current California RN licensure
  • Minimum of two (2) years of acute hospital clinical experience or
  • A Master's Degree in Case Management or Nursing field in lieu of one (1) year experience
  • BLS through the Amercian Heart Association (AHA) within 90 days

Preferred:
  • BSN degree or experience equivalent
  • Accredited Case Manager (ACM) certification
  • At least five (5) years of nurisng experience
