Job Description:

We are seeking an Experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic Hospitalist Team. In this role, you will be responsible for providing high-quality, patient-centered care to hospitalized patients across various specialties. You will work in a collaborative, fast-paced environment, assisting with the management of acute and chronic medical conditions, ensuring that patients receive the best possible care throughout their hospital stay.

Key Responsibilities:
  • Perform comprehensive physical examinations and take medical histories of hospitalized patients.
  • Collaborate with attending physicians, specialists, and hospital staff to develop and implement individualized treatment plans.
  • Monitor patient progress, adjust treatment plans as necessary, and ensure continuity of care from admission to discharge.
  • Order, interpret, and follow up on diagnostic tests and laboratory results.
  • Provide ongoing assessment, diagnosis, and management of patients with a wide range of medical conditions, including but not limited to respiratory infections, cardiovascular disease, diabetes, and post-surgical complications.
  • Document patient care accurately and thoroughly in electronic health records (EHR) in a timely manner.
  • Provide patient and family education on medical conditions, treatments, and care plans.
  • Respond to medical emergencies, including performing necessary interventions and managing acute situations.
  • Coordinate with multidisciplinary teams to ensure optimal patient care and smooth transitions of care.
  • Participate in hospital rounds, case discussions, and care planning meetings.
  • Assist in discharge planning and provide patients with appropriate post-discharge care instructions.
