Assistant Director of Nursing (ADON) - Risk Management & Education

Salary: Up to $130,000 per year

Water's Edge Rehab & Nursing Center at Port Jefferson is seeking an experienced and dedicated Assistant Director of Nursing (ADON) - Risk Management & Education to join our clinical leadership team.

This is an excellent opportunity for an experienced RN leader who is passionate about resident safety, staff development, clinical quality, and regulatory compliance. The ADON will work closely with the Director of Nursing and interdisciplinary leadership team to identify and mitigate clinical risks, strengthen nursing education, and promote the highest standards of resident care.
Responsibilities
  • Assist the Director of Nursing with the daily clinical operations of the nursing department.
  • Oversee facility-wide risk management initiatives and resident safety programs.
  • Review and investigate incidents, accidents, adverse events, and other potential areas of clinical risk.
  • Identify trends and develop corrective action plans to prevent recurring incidents.
  • Monitor quality indicators, including falls, pressure injuries, medication errors, infections, hospitalizations, and other clinical outcomes.
  • Assist with QAPI initiatives and performance improvement activities.
  • Conduct regular clinical audits and identify opportunities for improvement.
  • Develop, coordinate, and deliver education and competency programs for nursing staff.
  • Provide orientation and ongoing education to nurses and CNAs.
  • Ensure staff education is aligned with regulatory requirements, facility policies, and current clinical standards.
  • Maintain accurate records of employee education, competencies, and required training.
  • Collaborate with Nursing, Infection Prevention, Staff Development, and interdisciplinary team members.
  • Review clinical documentation and care practices to ensure compliance and quality.
  • Assist with state and federal surveys and maintain continuous survey readiness.
  • Ensure compliance with CMS, New York State Department of Health, OSHA, and facility requirements.
  • Provide clinical guidance and mentorship to nursing staff.
  • Assist with developing and updating nursing policies and procedures.
  • Support leadership with staff performance and clinical competency concerns.
Qualifications
  • Current and active New York State Registered Nurse (RN) license.
  • BSN preferred.
  • Previous ADON, Nurse Manager, Staff Development, Risk Management, or nursing leadership experience strongly preferred.
  • Skilled Nursing Facility, Long-Term Care, or Rehabilitation experience preferred.
  • Strong knowledge of CMS and New York State Department of Health regulations.
  • Experience with risk management, incident investigation, QAPI, and clinical audits.
  • Strong staff education and presentation skills.
  • Excellent communication, leadership, organizational, and critical-thinking abilities.
  • Ability to identify clinical risks and implement effective solutions.
  • Strong commitment to resident-centered care and quality outcomes.
