As the
Assistant Director of Case Management, you'll partner with hospital and departmental leadership to guide the daily operations of Case Management while supporting patient flow, utilization management, discharge planning, and interdisciplinary care coordination. Through strong people leadership, data-driven decision-making, and collaboration across clinical and community partners, you'll help remove barriers to care, strengthen team performance, and advance consistent practices that support patients throughout their healthcare journey.
As an Assistant Director of Case Management, a typical day may include:- Providing leadership and operational oversight for Case Management services while promoting high-quality, patient-centered care and adherence to clinical and regulatory standards
- Managing daily staffing, assignments, workflows, and resource allocation to support efficient department operations and patient care delivery
- Designing and optimizing staffing models and workflows to strengthen patient throughput, team effectiveness, and resource utilization
- Proactively identifying barriers to patient progression and collaborating with interdisciplinary partners to reduce avoidable delays and support appropriate length of stay
- Partnering with physicians and members of the care team on utilization review escalations, medical necessity, discharge planning, and complex care coordination needs
- Reviewing key performance indicators, including length of stay, readmissions, cost per case, and resource utilization, to identify trends and drive performance improvement
- Coaching, mentoring, and developing Case Management team members through ongoing feedback, performance management, education, and professional development
- Participating in recruitment, selection, onboarding, and retention efforts to build and maintain a knowledgeable, collaborative, and engaged Case Management team
- Supporting the development, implementation, and evaluation of policies and procedures to maintain compliance with regulatory requirements and Case Management best practices
- Collaborating with physicians, nurses, social workers, therapists, payers, community agencies, and other partners to coordinate care and support effective transitions
- Integrating social drivers of health and equitable care considerations into Case Management strategies, workflows, and patient care planning
- Partnering with leaders across the health system to align local operations with enterprise goals and advance standardization of effective Case Management practices

To be considered for this Assistant Director of Case Management opportunity, candidates must possess a Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW) and hold a current, valid license to practice as either a Registered Nurse (RN) or Licensed Social Worker in the state of New Jersey. For RN candidates, a master's degree in nursing, healthcare administration, or a related field is preferred. For Social Work candidates, Licensed Clinical Social Worker (LCSW) licensure is preferred. Certification in Case Management through ACM or CCM is strongly preferred and must be obtained within one year of hire. Candidates must possess a minimum of four years of clinical experience in a hospital, home health, or community setting, including at least two years of acute care Case Management experience and two years of supervisory experience involving multi-unit or program leadership. The ideal candidate demonstrates expertise in utilization management, medical necessity criteria, discharge planning, healthcare reimbursement, regulatory compliance, patient flow, performance improvement, data analysis, and interdisciplinary care coordination, along with the ability to lead, mentor, and inspire teams. This is a salaried day shift leadership opportunity supporting the Case Management department at the Williamson Street location.
