Lead Clinical Excellence and Help Shape the Future of Ambulatory Pharmacy Care

We are seeking an experienced Ambulatory Team Leader Pharmacist to provide clinical leadership, operational oversight, and advanced pharmaceutical care within our ambulatory pharmacy services. In this role, you will combine direct patient care responsibilities with leadership and mentorship opportunities, helping ensure high-quality, patient-centered care while supporting the growth and development of pharmacy team members.

As a clinical leader, you will work closely with physicians, advanced practice providers, pharmacists, technicians, and other healthcare professionals to optimize medication therapy, improve patient outcomes, and advance ambulatory pharmacy services. This position is ideal for a pharmacist who enjoys leading teams, improving processes, mentoring others, and practicing at the top of their license.
What You'll Do
  • Provide comprehensive medication management and advanced clinical pharmacy services for ambulatory patients
  • Design, monitor, evaluate, and optimize patient-specific pharmacotherapy plans
  • Collaborate with physicians and interdisciplinary care teams to improve patient outcomes
  • Provide patient education and medication counseling
  • Support services such as anticoagulation management, medication reconciliation, pharmacokinetic dosing, and other ambulatory pharmacy programs
  • Respond to medication-related questions and provide drug information to patients and healthcare providers
  • Participate in quality improvement initiatives and identify opportunities to enhance patient care, operational performance, and cost savings
  • Serve as a clinical resource and mentor for pharmacists, technicians, and Medical Office Assistants
  • Provide day-to-day leadership, oversight, and training to pharmacy team members
  • Assist with onboarding, orientation, and ongoing professional development activities
  • Precept pharmacy students and residents as appropriate
  • Contribute to educational programs through presentations, in-services, newsletters, and community outreach initiatives
  • Support compliance with all regulatory, accreditation, and professional practice standards
  • Contribute to departmental financial performance, operational effectiveness, and strategic growth initiatives
  • Maintain pharmacy systems, records, formularies, and medication-use processes in accordance with organizational standards
What We're Looking For
  • Bachelor of Science in Pharmacy (BSPharm) or Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
  • Licensed or eligible for licensure in the applicable state(s) of practice
  • Ability to obtain required licensure within 45 days of employment
  • Hospital pharmacy experience required
  • Advanced pharmacy certification required (such as BCPS, BCACP, BCOP, CACP, MBA, or other relevant advanced credential)
  • Strong clinical knowledge and commitment to evidence-based patient care
  • Excellent communication, leadership, and relationship-building skills
  • Ability to collaborate effectively with interdisciplinary healthcare teams
  • Strong problem-solving and decision-making abilities
  • Commitment to mentoring, staff development, and continuous improvement

Preferred:
  • Previous leadership or direct supervisory experience
  • Experience leading ambulatory care, clinical pharmacy, or medication management programs
  • Experience with quality improvement and performance management initiatives
