Job Location

We are seeking a Advanced Practice Provider to join one of the nation's most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. This opportunity is located at our Washington, Family Medicine Practice, at 7910 East Washington Street Indianapolis, IN 46219.

Highlights

Community Health Network has an excellent opportunity for a family medicine trained Advanced Practice Provider to work at our outpatient Washington, Family Care Medicine practice.

Experience
  • Family Medicine experience as an Advanced Practice Provider is preferred
  • Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.
  • Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
