Advanced Practice Provider - Hospital Medicine

Jones Memorial Hospital (JMH) is seeking a Nurse Practitioner or Physician Assistant to join our Hospital Medicine team. The APP Hospitalist provides inpatient medical care in collaboration with Hospital Medicine physicians and the interdisciplinary care team, including admission, ongoing management, cross-coverage, discharge coordination, and response to changes in patient condition.

Experienced APPs and new graduates are encouraged to apply.

Position Responsibilities
  • Provide inpatient medical care to assigned patients in collaboration with Hospital Medicine physicians and the interdisciplinary care team.
  • Obtain appropriate sign-out and ensure continuity of care throughout the assigned shift.
  • Evaluate and admit patients from the Emergency Department, ICU, and direct admission pathways, including obtaining comprehensive histories and performing physical examinations.
  • Develop and implement diagnostic and treatment plans in collaboration with the attending or covering physician.
  • Order and manage appropriate medications, diagnostic studies, therapies, and other clinical interventions within scope of practice and granted privileges.
  • Manage and cross-cover hospitalized patients, promptly evaluating changes in clinical condition and coordinating care with the covering physician as appropriate.
  • Complete timely and accurate admission, progress, discharge, and other required medical record documentation.
  • Facilitate patient flow, discharge planning, and transitions of care in collaboration with the interdisciplinary team.
  • Assist with advance care planning, including advance directives, code status, and DNR orders, in accordance with hospital policy and scope of practice.
  • Participate in emergency response situations, including Code Blue, and remain available in the hospital to respond promptly to patient care needs during assigned shifts.
  • Perform approved bedside procedures and obtain appropriate informed consent based on training, demonstrated competency, scope of practice, and granted privileges.
  • Participate in interdisciplinary huddles, quality assurance, quality improvement, patient safety, and other Hospital Medicine initiatives.
  • Maintain required licensure, certification, continuing education, credentialing, clinical privileges, and compliance with applicable hospital and regulatory requirements.

Practice Environment

The Hospital Medicine service operates through a collaborative physician-APP model. APPs work closely with Hospital Medicine physicians and the interdisciplinary team in the management of hospitalized patients.

Jones Memorial Hospital maintains an open ICU, providing opportunities to care for and collaborate across varying levels of inpatient acuity.

Qualifications

Candidates must meet the applicable education, licensure, certification, credentialing, and privileging requirements to practice as a Nurse Practitioner or Physician Assistant in New York State.

Nurse Practitioner:
  • Graduate of an accredited Nurse Practitioner program.
  • Current or eligibility for New York State licensure and registration as a Nurse Practitioner.
  • Current national board certification through an appropriate certifying organization.
  • Education and certification appropriate for the patient population and scope of the Hospital Medicine position

Physician Assistant:
  • Graduate of an accredited Physician Assistant program.
  • Current or eligibility for New York State licensure and registration as a Physician Assistant.
  • Current NCCPA certification.

All Candidates:
  • BLS and ACLS certification required, or ability to obtain within the timeframe established by Jones Memorial Hospital.
  • Ability to meet Jones Memorial Hospital Medical Staff credentialing and privileging requirements.
  • Strong clinical assessment, communication, documentation, and interdisciplinary collaboration skills.
  • Ability to respond appropriately to changes in patient condition and emergency situations.


New graduates are encouraged to apply. Previous hospital medicine, inpatient, critical care, emergency medicine, or other acute-care experience is preferred but not required. New graduate candidates will be considered based on education, clinical rotations, demonstrated competencies, and readiness for inpatient practice.
